Age UK Hertfordshire is an independent local charity working to, alleviate loneliness and isolation, tackle poverty and build resilience in the community for older people in Hertfordshire who need our help.
In 2018/19 our Information and Advice Service received over 300 calls a week, and each year for the last 4 years we have helped people claim over £ 6.3 million in benefits entitlements. Other county-wide services include eighteen day centres, a befriending scheme that supports older isolated people throughout Hertfordshire (providing 58,000 hours of support each year) a telephone befriending scheme (5,000 hours) and a free confidential support line for older people facing a life changing or challenging situation. None of these services are fully funded by external agencies, and so we are continually looking for additional funding to keep them operating.
We are the Lead Partner for Hertswise, a community dementia service, supporting people with memory impairment and their carers. We are also one of seven partners delivering the Hospital and Community Navigation Service, who help patients recently discharged from hospital or those in the community who need support to manage their health and well-being.
We provide 5-day a week services at our specialist day centre for people with dementia in Rickmansworth called Chalmers Court and at our Clitheroe Health and Well Being Centre in South Oxhey (which includes a Men in Sheds scheme).
We are also now working with Herts Community NHS Trust in the delivery of the Nutrition and Dietetics Partnership Service in West Hertfordshire.
